Registration
Please review the following information carefully to ensure that your child is properly registered for Calvary Hourly Childcare and to determine the applicable rates.
Fees
Registration fees are due by the first day your child visits Calvary Hourly Childcare:
$25 non-refundable registration fee (one-time fee)
-OR-
$40 non-refundable registration fee for those with permanent reservations (annual fee)

Forms
In addition to submitting a registration fee, you must complete a set of forms by your child’s first day at Calvary Hourly Childcare. Either pick up a packet at Calvary, or print out and complete the following:
- Registration form
- Emergency procedures & permission form
- Emergency card
- Immunization form
- Tuition Express form
- Health Care form (must be signed by your physician)
By the second time your child visits us, we must have a completed Health Care Summary signed by your child’s doctor.
Food Allergies – If a child has a food allergy, a Food Allergy Action Form will need to be completed by the child’s health care provider and must contain the specific food allergy and appropriate action plan. Please see Allergy Prevention and Response policy.
Rates
Our rates for the 2022-2023 school year are:
Toddler (16 months to 32 months): $12.70/hour
Preschooler (33 months to Kindergarten): $11.35/hour
Payment
Payment is accepted using Tuition Express, part of our center’s software management system. Once enrolled in Tuition Express, your tuition and fee payments will be paid automatically on the last day of the month. Please print the form, complete and attach a voided check and turn into the office. Learn more.
